Every Class I device recall in the past 14 months was voluntary
A Class I recall is the FDA's highest-severity classification, reasonable probability of serious injury or death. None of the 122 distinct recall events since July 2025 were FDA-mandated; every one was firm-initiated.

FY2026 Q4 (Jul–Sep 2026, in progress) shows only 4 so far, almost certainly a posting lag rather than a real drop, the same pattern warning letters show near the edge of the dataset.
A firm-initiated recall runs on the same discipline as CAPA
None of these 122 events waited for the FDA to force a recall. That is the same discipline a CAPA program is supposed to produce: a nonconformance detected and acted on before an outside body has to step in.

What to check: a firm that has filed more than one Class I recall in this window has already shown its own detection and correction process works, the open question is whether the underlying root cause was actually closed or just the specific product lot.
